One aspect to look at are the 'leading causes of death', which compared with white folks is much higher. That is heart disease, cancer, homicide & diabetes. Those are some rather deadly things! So obviously of they occur much more in black folks, it would make sense that overall the life expectancy can be lower. Then apparently from the 90's on HIV had its impact. So there are more medical reasons at play here than anything.

BUT - with that said, the standard of care is at play here too. How much for sure I can't say, but I know that it makes sense that if someone lives in a community with less money and they have less money, it stands to reason that they may not receive the same level of care.
And then I have seen doctors and/or offices treat patients differently. I have seen personnel act differently with black people and were rude. I have seen doctors get short with black people, or offer treatment options that that are less than what one would expect. One can only see so much as an independent outside witness. But I've had my share of hospital visits over the years, and planned treatments/surgeries, many many office visits, then add in dental visits or anything else considered medical. My ass has been there. So can't say for sure it then applies to the entirety based on what I witnessed, but that and man many stories tend to give ya an idea.

Chicago Mayor Lori Lightfoot (D) declared racism a “public health emergency” on Thursday and promised to divert $10 million in coronavirus relief to create “equity zones,” the Daily Mail reported.
“Ladies and gentlemen, it is literally killing us,” Lightfoot said during a news conference in front of the an exhibit honoring Martin Luther King Jr. in the West Side of Chicago.
Lightfoot made the announcement following a report from the city public health department, which found that non-black residents live to be 80.6 years on average, while black residents on average live 71.4 years.
